If hospitals successfully collect and submit patient-reported outcomes and limited risk variable data, they will receive two additional points towards their composite quality score. Two points is equal to 10% of the quality score, which is also made up of HCAHPS and complications. The composite quality scores are used to bucket hospitals into one of four quality categories, ranging from "below acceptable" to "excellent". Hospitals in the "good" or "excellent" categories are eligible for a quality incentive payment in the form of a reduced discount rate (1 - 1.5%). Two additional points can lead to a higher quality category resulting in either a higher reconciliation payment or less repayment responsibility. For one of our clients, voluntary submission of PRO data could translate to $95,000 in potential savings, because it is projected to move them from "good" to "excellent." There is money left on the table when hospitals pass on PRO. Here are a few things you need to understand when considering PRO in CJR: Which patients are eligible? Medicare beneficiaries aged 65 or older undergoing elective primary THA / TKA procedures. It is important to note that this is a subset of the larger CJR patient population. How many patients need to complete surveys? In year one, hospitals must submit pre-operative data on at least 50% or 50 eligible cases (the lesser of the two). This number grows to 85% or 200 eligible procedures in years 4 and 5. Starting in year two, post-operative data for patients from the previous year must be submitted in addition to the current year's pre-operative data. To get credit for post-operative, you must collect paired scores. A patient's pre-operative data may be captured 90 to 0 days prior to surgery (or Apr 1 – Aug 31, 2016 for year 1) and post-operative data must be captured 270 – 365 post surgery (or Apr 1 – Aug 31, 2017 for year 1 procedures – to be submitted year 2). When is the performance period? For performance year 1, patients who have a procedure July 1 – August 31, 2016. Remember... you have to collect your pre-operative outcomes data prior to that procedure, so you should be collecting data this month to meet the requirement. The procedure window for year two pre-operative data begins September 1, 2016. What data needs to be collected? The full dataset consists of 1 generic and 1 disease-specific PRO survey (PROMIS-10 and HOOS / KOOS JR) along with 7 risk variables and several patient identifiers. Where do hospitals submit the data to CMS?
